BLUE CIRCLE HEALTH IS A 501(C)(3) NONPROFIT ORGANIZATION THAT PROVIDES FREE VIRTUAL CLINICAL CARE, EDUCATION, AND SUPPORT FOR ADULTS WITH TYPE 1 DIABETES. BEYOND DIRECT CARE, BLUE CIRCLE HEALTH PARTNERS WITH CLINICIANS TO LEARN, DEMONSTRATE, AND SHARE SCALABLE MODELS THAT CLOSE CARE GAPS, PROVING THAT MORE EQUITABLE, EFFECTIVE T1D CARE IS POSSIBLE NATIONWIDE. DESIGNED BASED ON NEED RATHER THAN TRADITIONAL REIMBURSEMENT RULES, THE ORGANIZATION'S WHOLE-PERSON MODEL CAN INCLUDE ENDOCRINOLOGY CARE, DIABETES EDUCATION, INSURANCE NAVIGATION, PEER SUPPORT, DIABETES SUPPORTIVE COUNSELING, ASSISTANCE PAYING FOR T1D MEDICATION AND SUPPLIES, AND REFERRALS TO COMMUNITY RESOURCES THAT CAN HELP WITH ACCESS TO HOUSING, EMPLOYMENT AND FOOD.
