PACE SOUTHEAST MICHIGAN (PACE) IS AN ALTERNATIVE TO NURSING HOME CARE FOR THE FRAIL AND ELDERLY. THE INTERDISCIPLINARY TEAM AT PACE ASSESSES POTENTIAL PARTICIPANTS AND DEVELOPS A PERSON-CENTERED CARE PLAN - CUSTOMIZING SERVICES TO MEET INDIVIDUALS' NEEDS. FREQUENT CONTACTS WITH PARTICIPANTS AND REGULAR REVIEWS OF THE CARE PLAN ENSURE THAT PARTICIPANTS' NEEDS ARE BEING MET. PACE SEEKS TO FORM A PARTNERSHIP WITH THE PARTICIPANT AND CAREGIVERS TO PROMOTE INDEPENDENCE AND QUALITY OF LIFE.
