During fiscal year 2025, our services were provided to 406 individuals.Mental Health Case Management Program - We are a CARF-accredited outpatient behavioral health program with 28 years of experience providing high-quality rehabilitation services. Our focus is on intensive case management and community-based recovery support to help individuals achieve stability, wellness, and independence.Basic Housing Assistance Program - This program provides assistance with past-due rent & utilities to households in the Palmer area, including Chickaloon and Sutton, who are at imminent risk of homelessness due to the potential loss of their permanent residence.Navigation Center - This program provides resource navigation to households in the Palmer area, including Chickaloon and Sutton, who are at imminent risk of homelessness.Traumatic Acquired Brain Injury (TABI) - We provide compassionate, specialized support for individuals who have experienced a TABI and are navigating the challenges of a diagnosed mental illness, including resource facilitation, medical & rehab services, housing & transportation, and employment & education.CRP Case Management - The Palmer Coordinated Resources Project (CRP) is a specialized therapeutic or problem-solving court which serves individuals in the Mat-Su Borough who have been charged with misdemeanor or low-level felony offenses and are living with mental health disorders or disabilities.Mat-Su Reentry Program - We have been providing reentry case management services for individuals returning to the Mat-Su Borough from incarceration since the launch of the first pilot project in 2016, as part of the Mat-Su Reentry Coalition.
