THE ORGANIZATION SERVED APPROXIMATELY 4,058 MEMBERSMONTHLY UNTIL THE ORGANIZATION'S CONTRACT WITH CMS ENDED JULY 1, 2018. DURING 2021, THE ORGANIZATION CONTINUED WINDING DOWN OPERATIONS AS IT SATISFIED THE REMAINING OBLIGATIONS UNDER THE CMS CONTRACT AND THE COMPLETION OF OTHER ADMINISTRATIVE CLOSE OUT RESPONSIBILITIES.
