Provided 24-hour personal care and supportive residential services in a single family-like home setting serving up to three clients at a time. Services included assistance with activities of daily living (bathing, dressing, grooming, mobility, toileting, and eating), ensuring regular medication taking and reminders, meal planning and preparation, housekeeping, laundry, social/recreational activities, and involvement with implementing and providing feedback for individual's medical care plan through communication and coordination with clients' doctors, nurses, and home health agencies - all overseen and coordinated by a full-time staff RN. The program also provided daytime care for two individuals, one who later transitioned into full-time 24-hour residential care. Care was tailored to meet individual needs for short-term, long-term, rehabilitation, and end-of-life hospice care. During the tax year, the program served five unique clients and provided 365 days of residential personal care to clients. The primary objective was to offer a safe, dignified, home-like environment that supports clients' physical, emotional, social, and spiritual well-being, promoting recovery and independence where possible, and providing compassionate care at all phases of need, enabling individuals to remain in a community-based setting rather than more institutional environments.
