Hong Fook Adult Day Health Care - Hong Fook Community-Based Adult Services serve a vulnerable population of frail elderly and adults with chronic medical or mental health conditions, cognitive impairment, and/or disabilities. The mission of Hong Fook is to promote the optimal physical, emotional, and social well-being of our Participants and their caregivers; to minimize ER visit, hospitalizations, and to delay or prevent nursing home placement. Our locally based services play a vital role in Home and Community Based Services. They include individualized, comprehensive, and culturally sensitive services as follows: Professional nursing care and health education Chronic disease management and prevention Dementia care Mental health services Medical social services and case management Cognitively stimulating therapeutic activities Recreational, educational, and cultural activities Occupational, physical, and speech therapy services Registered dietitian services Personal care, transportation, and nutritious meals for participants Training, support, respite, and counseling to Participants' families and caregivers.Hong Fook Centers adopt a Person-Centered model of care which emphasizes meeting the individuals unique needs. Participants and their families are integrally involved in the care planning process which includes identifying their goals and accomplishing them through a strength-based approach. Our in-center services and emergency remote service ensure the continuity of care of our Participants. As an essential part of the Home and Community Based Services, Hong Fook Center continues to collaborate with a wide range of community, health care agencies to meet the changing needs of our Participants and Caregivers; and provide them with updated community resources and news. Our outreach team also collaborates closely with community organizations, government agencies, hospitals, and clinics to identify individuals in need of our services. Hong Fook Adult Day Health Care serves African American, Korean and Chinese Participants. The center offers morning and afternoon sessions each day.

Community Health Home (aka Housing and Community Services) supported the community through four key programs. (1) Community Mediators in Oaklands Chinatown and Downtown helped build safety by de-escalating 39 tense situations, conducting 1482 outreach interactions with unhoused individuals, and offering 1210 resource referrals as part of a statewide emergency response pilot. (2) Stop the Hate raised awareness about bias and discrimination through 16 social media campaigns, 20 community events, partnerships with two schools, three community gatherings, empathy tours, conference participation, and bias education at public events. (3) Kaiser Reentry Support helped ensure 58 individuals returning from incarceration were connected to health care and provided backpack of necessities, social service navigation, and housing support. (4) Oak Street Community Cabin successfully helped 27 residents transition into stable housing before the site was decommissioned at the end of 2024.
