Intensive Case management - HPP utilizes a holistic approach to empower pregnant women and families through trauma-informed case management services that help families work toward goals with one-on-one counseling (named internally ICM). Families who receive ongoing case management are generally involved with one of the following services at HPP: New Beginnings: Comprehensive support for women with high-risk pregnancies, including those with contributing risk factors such as homelessness, mental health issues, domestic violence and other challenges. Keeping Families Together: Individualized support for families involved with child welfare and/or substance use disorders. Bringing Families Home: Specialized housing assistance and counseling for high-need homeless families either involved with child welfare or at-risk of involvement. Domestic Violence Services: Family-centered support for those struggling with intimate partner violence. Mental Health & Parenting Services: Onsite individual, couples, family and group therapy to HPP clients, as well as parenting classes, fatherhood services and childrens playgroups. Specialized services for families and young children who have witnessed community or family violence (offered onsite through the Child Trauma Research Project).

Housing Assistance Program services include housing search and placement, financial assistance with move-in costs, lifting barriers to obtaining housing, budget counseling and short-term rental subsidies. HPP also offers regularly Housing Workshops to help educate families on housing processes (including search, tenants rights and responsibilities, etc.).

Transitional Housing Located in the Bayview District, Jelani House is a transitional living facility for up to 17 pregnant, homeless women. Residents stay at Jelani House at any point in their pregnancy, through the birth of their babies and into their postpartum bonding/healing period. While at Jelani House, residents receive case management, and are able to access groups, classes, therapy and job training. HPP also helps these families exit to permanent, safe and long-lasting housing.
