BCH CLINICAL WAS FUNDED INTO EXISTENCE BY THE LEONA M. AND HARRY B. HELMSLEY CHARITABLE TRUST. THE ORGANIZATIONS PARTNER WITH CLINICIANS TO LEARN, DEMONSTRATE, AND SHARE SCALABLE MODELS THAT CLOSE CARE GAPS, PROVING THAT MORE EQUITABLE, EFFECTIVE T1D CARE IS POSSIBLE NATIONWIDE. DESIGNED BASED ON NEED RATHER THAN TRADITIONAL REIMBURSEMENT RULES, BCH CLINICAL'S WHOLE-PERSON MODEL CAN INCLUDE ENDOCRINOLOGY CARE, DIABETES EDUCATION, INSURANCE NAVIGATION, PEER SUPPORT, DIABETES SUPPORTIVE COUNSELING, AND REFERRALS TO COMMUNITY RESOURCES THAT CAN HELP WITH ACCESS TO HOUSING, EMPLOYMENT, FOOD, AND ASSISTANCE PAYING FOR T1D MEDICATION AND SUPPLIES.
