CLINICAL OPERATIONS INCLUDE INDIVIUDAL CASE MANAGEMENT, CONSULTATION WITH PATIENT'S ADMINITTING PHYSICIAN, CREATING PLANS OF CARE, CREATING AND MONITORING PERSONALIZED PATIENT DIETS, CONSLUTING ON PHARMACEUTICAL EFFECTS AND INTERACTIONS AND CONDUCTING ONGOING CLINICAL TRAINING FOR EMPLOYEE THAT PROVIDE HANDS-ON CARE. CHSI IS THE SECTION 509 (A)(3) SUPPORTING ORGANIZATION AND PARENT ENTITY OF THE INTEGRATED HEALTH CARE DELIVERY SYSTEM OF WHICH THE ORGANIZATION IS A MEMBER. THE IHDS PROVIDES SUBSTANTIAL COMMUNITY BENEFIT TO THE COMMUNITIES IT SERVES AND CONTINUES TO ASSESS AND DEVELOP PROGRAMS TO BENEFIT THE COMM
